CareGivers America – NEPA, a Modivcare Home Care company, is hiring a Full-Time RN Case Manager to provide one-on-one pediatric care in the home setting throughout parts of East Lackawanna, Wayne, Wyoming, and Susquehanna counties.

This is a salaried Monday–Friday role with a company vehicle provided.

You will manage care for pediatric patients in their homes, serving as the clinical lead and trusted partner to families.

Branch Location: 718 S. State Street, Clarks Summit, PA 18411
Territory: East Lackawanna County, Wayne County, Wyoming County, and Susquehanna County
Schedule: Full-Time | 40 hours per week | Monday–Friday

Your Impact as a Pediatric RN Case Manager

• Complete comprehensive patient assessments, including OASIS data collection, within established timelines
• Develop and oversee individualized pediatric care plans in collaboration with physicians and families
• Provide clinical direction and supervision to LPNs and Home Health Aides
• Ensure documentation accuracy, timeliness, and regulatory compliance
• Communicate consistently with families and interdisciplinary care teams
• Participate in quality improvement initiatives and staff education
• ​​​​​​​Maintain a safe, respectful, and supportive care environment

Qualifications

• Active Pennsylvania RN license (Required) – BSN preferred
• Minimum 2 years of professional nursing experience
• Pediatric or home health experience preferred
• Strong leadership and communication skills
• Ability to work independently and manage multiple priorities
• Valid driver’s license
• ​​​​​​​Current CPR certification
