RN Case Manager – Pediatric Home Health (Field-Based)

CareGivers America – NEPA, a Modivcare Home Care company, is hiring a Full-Time RN Case Manager to provide one-on-one pediatric home health visits across:

  • Schuylkill County
  • North Berks County
  • South Northumberland County
  • Carbon County

This is a salaried, field-based role with a company vehicle provided.

What to Expect

  • 40 hours per week
  • Five days per week (includes weekdays and weekends)
  • Daily travel within assigned coverage counties
  • Salaried position — not per-visit pay

Nurses travel throughout the listed counties based on patient needs and scheduling.

Qualifications

  • Active Pennsylvania RN license
  • Minimum 2 years of professional nursing experience
  • Pediatric or home health experience preferred
  • Strong leadership and organizational skills
  • Ability to work independently
  • Valid driver’s license
  • Current CPR certification
