Registered Nurse (RN) – Remote (Part-Time)
Care Finders Total Care LLC, a Modivcare Home Care Company – New Jersey

Care Finders Total Care, a Modivcare Home Care company, is seeking a Registered Nurse (RN) to manage a caseload of members and conduct 30-day patient satisfaction and quality assurance (QA) surveys via telephone. This role focuses on assessing members’ physical and psychosocial needs while maintaining accurate nursing documentation in our internal systems.

Responsibilities:

  • Conduct inbound and outbound calls to complete 30-day patient satisfaction surveys
  • Assess members’ physical and psychosocial status
  • Follow up on certified health conditions, current care plans, and program services
  • Document all interactions and findings in the internal database

Compensation & Schedule:

  • Pay: $39–$42 per hour (based on experience)
  • Schedule: Part-time, remote position (hours may vary based on business needs)

Requirements:

  • Active RN license in New Jersey
  • Minimum of 1 year of RN experience in the United States
  • Current CPR certification
  • Home care experience preferred

Bilingual (Spanish) Required
