Care Finders Total Care, a Modivcare Personal Care Service, is looking for an RN or LPN responsible for providing high-quality services to our home care private duty clients.
 

We are seeking a compassionate, reliable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide one-on-one nursing care for a 15-year-old teen with a seizure disorder. This role is ideal for a nurse who enjoys building meaningful connections, maintaining a calm presence, and supporting a teen’s safety, independence, and quality of life in a private setting.

Position Spotlight...

• Pay: LPN hourly rate $35  , RN hourly rate $40
• Schedule: 2 days per week Mon & Tues 7:30a-5p

Responsibilities

• Monitor for seizure activity and respond appropriately per established care plan

• Administer medications as prescribed and observe for side effects

• Maintain accurate documentation of care, seizures, and interventions

• Ensure patient safety during daily activities

• Provide calm reassurance and support during and after seizure episodes

• Communicate effectively with family, care team, and supervisors

• Support the teen’s routine, comfort, and dignity

• Follow physician orders and individualized care plan at all times

Qualifications

• Active LPN or RN license in good standing

• Experience with seizure disorders preferred (training considered for the right candidate)

• CPR/BLS certification

• Strong assessment and critical-thinking skills

• Dependable, punctual, and attentive

• Comfortable working independently in a private-duty setting

• Compassionate, patient, and respectful when working with adolescents
