About the Opportunity

All Metro Health Care, a Modivcare Personal Care Service, is seeking an experienced Per Diem RN Clinical Supervisor to support our home care operations throughout New York City.
This field-based opportunity is ideal for a Registered Nurse with direct experience in a New York State Licensed Home Care Services Agency (LHCSA) who understands home health aide supervision, home care regulations, and clinical documentation requirements.
In this role, you'll primarily conduct in-home supervisory visits to ensure Home Health Aides (HHAs) provide safe, appropriate care in accordance with established plans of care and applicable home care requirements.

You'll also support nursing assessments, reassessments, and additional clinical visits as needed.

We're looking for an RN who can work independently, has hands-on home care supervisory experience, and offers flexible availability for early-morning and evening visits across assigned NYC boroughs.

What You'll Do

  • Conduct in-home supervisory visits while assigned Home Health Aides are providing patient care.
  • Observe and evaluate caregiver performance, patient safety, and adherence to established plans of care.
  • Provide clinical guidance, education, and corrective instruction to HHAs as needed.
  • Conduct initial nursing assessments, reassessments, and post-hospitalization visits as assigned.
  • Review and update individualized plans of care in collaboration with the clinical team.
  • Identify and communicate changes in patient conditions, caregiver performance, or service needs.
  • Coordinate with branch staff and clinical management to ensure timely supervisory coverage.
  • Provide additional field support when regularly assigned nurses are unavailable.
  • Maintain accurate and timely clinical documentation in accordance with agency policies and applicable New York State home care regulations.
  • Support continuity of care through effective communication with patients, caregivers, and clinical leadership.

What We're Looking For

Required Qualifications

  • Active, unrestricted New York State Registered Nurse (RN) license in good standing.
  • Professional RN experience with direct exposure to LHCSA home care operations and supervisory responsibilities.
  • Experience conducting in-home HHA supervisory visits and evaluating caregiver performance.
  • Working knowledge of home care regulations, care plans, and clinical documentation requirements.
  • Current CPR certification.
  • Strong clinical assessment, communication, organizational, and patient education skills.
  • Ability to work independently and manage field assignments with minimal supervision.
  • Ability and willingness to travel across assigned NYC service territories.
  • Availability to conduct supervisory visits during early-morning hours (approximately 7:30 AM) and evening hours (after 8:00 PM), based on patient and caregiver schedules.

Preferred Qualifications

  • Experience conducting initial nursing assessments, reassessments, and post-hospitalization visits within a LHCSA.
  • Familiarity with HHAeXchange or similar home care documentation systems.
  • Experience providing caregiver education, competency evaluations, and clinical follow-up.
  • Flexibility to accept additional assignments across multiple NYC boroughs as operational needs arise.

Schedule & Service Area

  • Employment Type: Per Diem | Field-Based
  • Primary Service Areas: Manhattan, Bronx, and Brooklyn
  • Additional Coverage: Staten Island, as needed
  • Office Location: Manhattan, NY
  • Schedule: Flexible, visit-based assignments based on patient needs and operational coverage.
  • Early-Morning Availability: Supervisory visits may be scheduled around 7:30 AM.
  • Evening Availability: Supervisory visits may be scheduled after 8:00 PM.
  • Additional Visits: Occasional daytime or weekend assignments may be available based on operational needs.
  • Geographic Flexibility: Nurses must be comfortable accepting assignments across designated NYC boroughs when additional coverage is needed.

Important Scheduling Information

Many of our patients receive home care services through caregiver shifts operating from approximately 8:00 AM–8:00 PM and 8:00 PM–8:00 AM.
Supervisory visits must be coordinated while the assigned Home Health Aide is present and providing care. As a result, early-morning and evening availability is particularly important.

This is not a traditional weekday daytime nursing position. Visit assignments, schedules, and volume may vary based on patient needs and operational demand. A fixed number of weekly visits is not guaranteed.

Per Diem Compensation

Earn $60–$180 per visit, depending on visit type, plus mileage reimbursement.

  • Supervisory & Post-Hospital Visits: $60
  • Reassessment Visits: $90
  • Initial Admission Assessments: $120 weekdays | $180 weekends

Mileage Reimbursement: Provided at the current federal rate for eligible travel.

This is a per diem, visit-based position. Visit assignments and earnings vary based on patient needs and the types of visits completed.
