All Metro Health Care, a Modivcare Personal Care Service, is seeking a Per Diem RN Supervisor – Home Care to support our clients and caregivers in the Corning, NY area. This is an ideal opportunity for an experienced RN looking for flexible, per diem, field-based work while maintaining a strong clinical and leadership presence in home care.

Compensation – Pay Per Visit
- Supervisory / Post-Hospital Visit: $35–$60 per visit
Supervisory visits are typically brief (approximately 15–20 minutes) and include required documentation.
- Reassessment: $75 per visit
- Initial Admission: $90 weekday | $125 weekend
Schedule & Location
- Per diem role requiring availability 1–2 days per week
- Visits are scheduled based on client and caregiver availability
- Field-based role requiring in-home supervisory visits within the Corning, NY area
- Office location: 318 Park Avenue, Corning, NY 14830
- Office visits are limited to orientation, and meetings as needed
What You’ll Do
- Conduct on-site supervisory and teaching visits to evaluate caregivers
- Provide clinical orientation and ongoing guidance for assigned home care cases
- Complete initial nursing assessments and reassessments
- Participate in training and in-services for field staff
- Collaborate with patients, caregivers, and physicians to assess needs and adjust care plans
- Ensure accurate, timely clinical documentation and compliance
- Monitor patient condition changes and communicate appropriately
What We’re Looking For
- Registered Nurse (RN) – active NY license
- CPR Certified
- Minimum 1 year of home care experience
- 1 year of supervisory or management experience
- Valid NYS driver’s license, auto insurance, registration, and reliable vehicle
