About the Opportunity

All Metro Health Care, a Modivcare Personal Care Service, is seeking a Per Diem RN Clinical Supervisor to support patients and home health aides throughout Liverpool and surrounding communities.
This field-based opportunity is ideal for a Registered Nurse who enjoys working independently, building meaningful relationships, and helping patients receive safe, high-quality care in the comfort of their homes.
In this role, you'll conduct nursing assessments, perform supervisory visits, provide clinical guidance to caregivers, and collaborate with the care team to support patients' individual needs.

What You'll Do

• Conduct initial nursing assessments, reassessments, and supervisory visits in patients' homes. 
• Supervise, evaluate, and provide clinical guidance to home health aides. 
• Develop and update individualized plans of care in collaboration with physicians and the care team. 
• Educate patients and caregivers regarding nursing care and related needs. 
• Monitor patients' conditions and communicate changes to the appropriate care team members. 
• Maintain accurate and timely clinical documentation.

What We're Looking For

Required Qualifications

• Active New York State Registered Nurse (RN) license in good standing. 
• Minimum of 1 year of professional RN experience in a skilled-care setting. 
• Current CPR certification. 
• Valid New York State driver's license, auto insurance, vehicle registration, and reliable transportation. 
• Ability to travel throughout Liverpool and surrounding areas for assigned patient visits.

Preferred Qualifications

• Previous nursing experience in home care, home health, or community-based nursing. 
• Experience conducting patient assessments, care coordination, or supervisory visits. 
• Familiarity with home care documentation and caregiver oversight.

Schedule & Service Area

• Employment Type: Per Diem | Field-Based 
• Territory: Liverpool and surrounding communities 
• Schedule: Flexible, visit-based assignments 
• Visit Coordination: Supervisory visits are scheduled when the patient's assigned home health aide is present. 
• Travel: Throughout the assigned service territory.

Visit assignments are based on patient needs and operational demand. Per diem hours and visit volume may vary.

Per Diem Compensation

Earn $35–$125 per visit, depending on visit type, plus mileage reimbursement.

• Supervisory and Post-Hospital Visits: $35–$60 
• Reassessment Visits: $75
• Initial Admission Assessments: $90 weekdays | $125 weekends

Mileage Reimbursement: Provided at the current federal rate for eligible travel.
