Description

Care Finders Total Care, a Modivcare Personal Care Service, is looking for a Care Coordinator Supervisor. You will have the opportunity to make a positive impact on the lives of our patients by ensuring their care needs are met effectively and efficiently.

  • Pay: $22 - $25 Hourly, depending on experience 
  • Schedule: M-F 8:30AM-5:00PM

You will...

  • Supervise and support Care Coordinators, including assigning tasks, monitoring workloads, and ensuring compliance with policies and procedures.
  • Oversee patient intake and care coordination processes, ensuring continuity, quality of care, and timely service delivery.
  • Receive and review referrals from healthcare providers, ensuring documentation is complete and accurate.
  • Manage caregiver scheduling and staffing coverage based on authorized hours, caregiver skills, and client preferences.
  • Serve as the primary point of contact for patients, families, caregivers, and healthcare teams regarding care coordination needs.
  • Communicate patient updates, care plans, and scheduling changes while maintaining accurate and confidential records.
  • Participate in care conferences, resolve service barriers, and support billing and service verification processes.
  • Mentor and provide guidance to care coordination staff on complex cases and day-to-day operations.
  • Educate patients and families on available services, community resources, and care expectations.
  • Support quality improvement initiatives, data collection, compliance efforts, and participate in on-call rotation as needed.
  • Perform other duties as assigned.

We are excited to speak to someone with the following…

  • High school diploma or equivalent
  • 3+ years of care coordination experience required.
  • 1+ years of experience using a Home Care Platform or scheduling system such as HHA Exchange 
  • Bilingual in English & Spanish required
