CareGivers America, a Modivcare Company, is a regional provider of Non-Medical Home Care, Home Health and Private Duty Nursing services.
We have 22 offices in Pennsylvania, serving 40+ counties and we are continuing to grow.

Due to growth and expansion, we are seeking a Care Coordinator for our Agency With Choice division at our Jenkintown Office Location


Responsibilities:

  • Attend participant ISP Meetings to engage in the development of patient outcomes & goals.
  • Assist in training employees/surrogates in proper documentation according to outcomes in the ISP.
  • Review daily logs and provide re-training to employees as needed
  • Train employees upon hire and annually
  • Train surrogates upon Start of Care and as needed
  • Assist in review, completion and re-training of Monthly Progress Notes
  • Complete all applicable HCSIS incidents/events
  • Complete all applicable certified investigations (training to be provided, as needed)
  • Complete certified investigation peer reviews, as applicable.
  • Annually, send Surrogate and Employee Satisfaction Surveys, collect and analyze the data, send summary to ODP & SC entities
  • Manage relationships with SC entities
  • Assist in gathering data for Annual Provider Qualification and Monitoring
  • Attend Quality Council Meetings & actively participate in the provider network
  • Maintain relationships with SC entities, ODP representatives, and community resources, allowing the company to grow & develop Intellectual Disability Program
  • Coordinate in-services with SC entities to educate on company offerings
  • Review business operations and provide feedback to improve compliance with all program directives, bulletins, info memos, and regulations
  • As needed, work with the Agency with Choice Manager to complete the following duties timely & adequately:
    • Manage all consumer & employee schedules, as needed.
    • Ensure accurate and compliant telephony use by employees.
    • Ensure employees are complying with proper documentation of services provided.
    • Answer consumer, managing employer, and employee questions.
    • Give families access to online schedules.
    • Enter new consumer and employee data in scheduling system.
    • Confirm payroll.
    • Ensure accurate and timely completion and submission of monthly transportation mileage logs and progress notes, including billing to Promise.
    • Document all communication with supports coordinators on any consumer changes, issues, concerns, incidents, or questions.
    • Document communication with supports coordinators when consumers are under or over utilizing authorized services.
    • Become knowledgeable of the scheduling system, using it daily as the basis for all scheduling needs.
    • Troubleshoot any billing issues or over-utilization, acting as the liaison with the back office, the supports coordinators, and the managing employers.
    • Contact supports coordinators about under-utilization of services.
    • Manage and maintain consumer, managing employer, and employee files according to regulations.
    • Use the comment area in AppointMate to document all events, incidents, complaints, etc. that occur for both consumers and caregivers.
    • Assist with the collection of needed documentation and work with the back office to obtain payment for all AWC-related vendor services, including billing to Promise and check requests.
    • Assist with filing an injury claim for all injured workers through the Worker’s Compensation Program. Be aware of changes in the injured worker’s status and when released back to work, making efforts to find them suitable work.

Requirements:

  • Maintain knowledge of ODP regulations and implement all guidelines throughout job responsibilities
  • Attend staff and employee meetings as requested
  • Maintain confidentiality and abide by HIPAA regulations
  • Provide suggestions to improve the efficiency of the office
  • Assist in filing, copying, and administrative tasks
  • Manage routine office files, log books, and schedules Job Qualifications
  • High school diploma
  • Must possess positive, enthusiastic attitude, good people skills, and professional appearance
  • Ability to work independently without constant supervision
  • Experience working with the Behavioral Health/Intellectually Disabled Population

Job Type:

  • Full Time
  • On-Site with travel throughout Philadelphia and surrounding areas. A company car will be provided. 
  • Monday - Friday
