CareGivers America – NEPA, a Modivcare Home Care company, is seeking a skilled and mission-driven Registered Nurse to join our Home Health team as an RN Case Manager.

Territory: Pottstown, PA
Schedule: Full-time | 40 hours per week
Monday–Sunday | 11:00 AM – 8:00 PM (5 days per week, including weekdays and weekends)
Compensation & Benefits:
- Salary: $70,000 – $78,000

As an RN Case Manager, you will:
- Complete comprehensive patient assessments, including OASIS data collection, within established timelines
- Collaborate with physicians, other healthcare professionals, patients, and families to develop individualized care plans
- Provide direction and supervision to LPNs and Home Health Aides, ensuring quality and effective care delivery
- Monitor clinical documentation for accuracy, timeliness, and compliance with all regulatory standards
- Communicate regularly with patients, families, and care teams to coordinate services and address changing needs
- Participate in quality improvement initiatives and staff education
- Promote a safe and respectful care environment for patients and colleagues
We Are Excited to Speak to Someone With...
- Active PA RN License (Required) – BSN Preferred
- 2+ years of professional nursing experience (Home Health experience preferred)
- Strong leadership, communication, and organizational skills
- Ability to manage multiple priorities and work independently
- Valid driver’s license, reliable insured vehicle, and current CPR certification
