CareGivers America – NEPA, a Modivcare Home Care company, is seeking a skilled and mission-driven Registered Nurse to join our Home Health team as an RN Case Manager.

Territory: Pottstown, PA
Schedule: Full-time | 40 hours per week
Monday–Sunday | 11:00 AM – 8:00 PM (5 days per week, including weekdays and weekends)
Compensation & Benefits:

• Salary: $70,000 – $78,000

As an RN Case Manager, you will:

• Complete comprehensive patient assessments, including OASIS data collection, within established timelines
• Collaborate with physicians, other healthcare professionals, patients, and families to develop individualized care plans
• Provide direction and supervision to LPNs and Home Health Aides, ensuring quality and effective care delivery
• Monitor clinical documentation for accuracy, timeliness, and compliance with all regulatory standards
• Communicate regularly with patients, families, and care teams to coordinate services and address changing needs
• Participate in quality improvement initiatives and staff education
• Promote a safe and respectful care environment for patients and colleagues

We Are Excited to Speak to Someone With...

• Active PA RN License (Required) – BSN Preferred
• 2+ years of professional nursing experience (Home Health experience preferred)
• Strong leadership, communication, and organizational skills
• Ability to manage multiple priorities and work independently
• Valid driver’s license, reliable insured vehicle, and current CPR certification
