, a Modivcare Personal Care Service, is looking for a RN Field Supervisor responsible for ensuring quality and safe delivery of home health Care services such as supervising Caregivers/Certified Home Health Aides in a patient’s home, evaluate the patient’s plan of care and assess their physical and Psychosocial needs.

  • Schedule: M-F 8:30AM-5:00PM 
  • Visiting Client Territory: Brighton and surrounding areas 
  • Office:18R Shepard Street, Brighton, MA 02135
  • Pay Range: $43-$45/hourly rate range

You will...

  • Performs on-site supervisory and teaching visits to evaluate caregivers
  • Provides clinical orientation to caregivers on assigned home care cases. 
  • Participates in the training of new nurses and in-services for field staff.
  • Performs initial nursing assessments
  • Counsels patient/caregiver in meeting nursing and related needs.
  • Assigns and coordinates services provided.
  • Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs. 
  • Prepares clinical progress notes ensuring any changes in the patient’s condition is reported 
  • Monitors clinical records for complete and accurate documentation.
