Description

CareGivers America, a Modivcare Personal Care Service, is looking for an RN/LPN responsible for providing high-quality services to our home care private duty clients.
 

We are seeking a compassionate, attentive, and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide one-on-one care for a bright and resilient 12-year-old girl in her home. She is recovering from a significant neurological injury and requires ongoing medical support, monitoring, and assistance with daily activities.

This role is ideal for a nurse who is patient, observant, and confident in pediatric care, and who values building meaningful connections with their patient and family.

Primary Responsibilities

  • Provide skilled nursing care in accordance with physician orders and care plan

  • Perform ongoing neurological and physical assessments

  • Monitor vital signs and observe for subtle changes in condition

  • Administer medications and treatments as prescribed

  • Assist with mobility, positioning, and safety supervision

  • Support feeding needs and ensure adequate hydration/nutrition as directed

  • Provide assistance with activities of daily living (ADLs) as needed

  • Maintain accurate documentation of care provided and patient response

  • Communicate effectively with parents, physicians, therapists, and care team members

  • Promote a calm, supportive, and structured healing environment


Ideal Candidate

  • Current LPN or RN license in good standing

  • Pediatric experience preferred (home health experience a plus)

  • Experience with neurological recovery or complex medical needs is beneficial

  • Strong assessment and critical thinking skills

  • Gentle, patient, and emotionally supportive demeanor

  • Ability to remain calm and responsive in urgent situations

  • Reliable, professional, and respectful of family privacy
