CareGivers America – NEPA, a Modivcare Home Care company, is seeking a Full-Time RN Case Manager to provide one-on-one pediatric home health visits throughout Schuylkill, North Berks, South Northumberland, and Carbon counties.

This is a salaried, field-based position with a company vehicle provided. Nurses in this role manage an assigned local territory and provide consistent, relationship-based care to pediatric patients in the home setting.

This role is ideal for nurses seeking a structured 40-hour schedule, defined territory coverage, and relief from productivity-based pay models.

Position Details

• Full-Time, field-based role
• 40 hours per week
• Five days per week (includes weekdays and weekends)
• Territory-based routing designed to support manageable travel
• Company vehicle provided

Nurses travel daily within their assigned coverage area to provide skilled nursing care and case management services.

Key Responsibilities

• Complete comprehensive patient assessments, including OASIS data collection, within required timelines
• Develop and oversee individualized pediatric care plans in collaboration with physicians and families
• Provide clinical supervision and direction to LPNs and Home Health Aides
• Monitor documentation for accuracy, timeliness, and regulatory compliance
• Communicate regularly with patients, families, and interdisciplinary care teams
• Participate in quality improvement initiatives and staff education
• Promote a safe and supportive home care environment

Qualifications

• Active Pennsylvania Registered Nurse (RN) license
• Minimum of two years of professional nursing experience
• Home health or pediatric experience preferred but not required
• Strong leadership, communication, and organizational skills
• Ability to manage multiple priorities independently
• Valid driver’s license
• Current CPR certification
