RN Case Manager – Pediatric Home Health (Field-Based)

CareGivers America – NEPA, a Modivcare Home Care company, is hiring a Full-Time RN Case Manager to provide one-on-one pediatric home health visits across:

  • Susquehanna County
  • Wyoming County
  • East Lackawanna County

This is a field-based, salaried role with a company vehicle provided.

Schedule & Travel Expectations

  • Full-Time | 40 hours per week
  • 5 days per week
  • Monday–Sunday | 11:00 AM – 8:00 PM (Flexible)
  • Includes weekdays and weekends (Flexible)
  • Daily travel across assigned coverage counties based on patient needs

This role requires flexibility and comfort with travel within the listed counties.

What You’ll Do

  • Complete comprehensive patient assessments, including OASIS documentation
  • Develop and oversee individualized pediatric care plans
  • Collaborate with physicians, families, and interdisciplinary care teams
  • Provide supervision and direction to LPNs and Home Health Aides
  • Monitor documentation for compliance and regulatory standards
  • Participate in quality improvement initiatives
  • Maintain a safe and respectful care environment

You will serve as the clinical lead and primary point of contact for pediatric patients and their families.

Qualifications

  • Active Pennsylvania RN license (Required)
  • Minimum 2 years of professional nursing experience
  • Pediatric or home health experience preferred
  • Strong leadership and communication skills
  • Ability to manage multiple priorities independently
  • Valid driver’s license
  • Current CPR certification
