CareGivers America, a Modivcare Personal Care Service, is looking for an RN/LPN responsible for providing high-quality services to our home care private duty clients.

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide one-on-one care for a bright and engaging 21-year-old young adult with complex medical and developmental needs.

This position offers the opportunity to make a meaningful difference in someone’s daily life while working in a supportive, family-centered environment.

Responsibilities

• Provide skilled nursing care in a home setting

• Monitor and manage seizure activity according to care plan

• Administer medications as prescribed

• Assist with mobility, positioning, and activities of daily living

• Support communication and engagement in daily activities

• Maintain accurate documentation and communicate changes to family and providers

• Ensure safety, comfort, and dignity at all times
