Care Finders Total Care, a Modivcare Personal Care Service, is looking for an RN or LPN responsible for providing high-quality services to our home care private duty clients.
 

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide one-on-one care for a 4-year-old child in a home setting. Child is bright, curious, and playful, and requires skilled nursing support due to medical needs.

This role is ideal for a nurse who enjoys pediatric care and values building a trusting, long-term relationship with a child and family.

Position Spotlight...

• Pay: LPN hourly rate $36, RN hourly rate $43
• Schedule: 7 days per week 3p-11p

Responsibilities

• Provide skilled pediatric nursing care in the home

• Manage and care for a G-tube, including:

  • Administering feeds, hydration, and medications

  • Monitoring tolerance and reporting concerns

  • Routine site care and troubleshooting (per training/protocol)

• Monitor vital signs and overall health status

• Observe for and report any changes in condition

• Assist with daily routines as appropriate (playtime, transitions, rest)

• Maintain accurate documentation of care provided

• Communicate clearly and respectfully with parents/caregivers

• Follow the child’s individualized care plan and physician orders

• Support a safe, calm, and nurturing environment

Qualifications

• Current LPN or RN license (state-specific, active, and in good standing)

• Pediatric experience preferred (home care experience a plus)

• CPR/BLS certification (pediatric preferred)

• Reliable, patient, and attentive to detail

• Comfortable working independently in a home setting

• Background check and references required
