Registered Nurse (RN) Clinical Supervisor – Field Position (Home Care)
Bilingual Spanish Required | Full-Time | West New York, NJ
Care Finders Total Care LLC, a Modivcare Home Care company, is seeking a Full-Time RN Clinical Supervisor for our West New York, NJ branch.
This is a FIELD-BASED role that requires traveling to patients’ homes to supervise care, complete nursing assessments, and support caregivers. If you’re a Spanish-speaking RN who enjoys working in the community—not behind a desk—and wants a stable weekday schedule, this role may be a great fit.

Position Details
- Office Location: 443 60th Street, West New York, NJ 07093
- Work Environment: Field-based (patient home visits) with office support
- Schedule: Full-Time | Monday–Friday | 8:30am–5:00pm
- Salary: Up to $85,000 annually, based on experience
What You’ll Do
- Conduct in-home supervisory visits to patients’ residences
- Supervise and educate Certified Home Health Aides in the field
- Perform initial nursing assessments and ongoing evaluations
- Collaborate with physicians to develop and update plans of care
- Educate patients and caregivers on medical and nursing needs
- Administer medications and treatments as prescribed
- Review clinical documentation for accuracy and compliance
- Support field staff through mentorship, guidance, and training
What We’re Looking For
- Active New Jersey RN license (required)
- Bilingual – Spanish & English (required)
- Current CPR certification
- Home care, visiting nurse, or community health experience preferred
- Comfortable traveling locally to patient homes
- Strong communication, organization, and time-management skills
- Ability to work independently in the field while supporting a team
