Care Finders Total Care, a Modivcare Personal Care Service, is looking for an RN or LPN responsible for providing high-quality services to our home care private duty clients.
 

We are seeking a compassionate, skilled LPN or RN to provide one-on-one, private duty nursing care for a 23-year-old female with complex medical needs, The patient requires total care and continuous skilled nursing oversight in a home setting. The ideal candidate is patient-focused, detail-oriented, and experienced with ventilator-dependent and medically fragile patients.

Position Spotlight...

• Pay: LPN hourly rate $35 - RN hourly rate $45
• Schedule: Wednesday overnight 11p-7a

Responsibilities

• Provide comprehensive skilled nursing care in accordance with the care plan and physician orders

• Manage tracheostomy care, suctioning, and ventilator monitoring

• Administer medications, tube feedings, and flushes via gastrostomy tube

• Perform bowel and bladder care, including incontinence management

• Monitor vital signs and respiratory status; recognize and respond to changes in condition

• Maintain skin integrity and assist with repositioning and passive range-of-motion exercises

• Ensure infection control and proper handling of medical equipment

• Accurately document care provided and patient status

• Communicate effectively with family members, care team, and supervising providers

• Maintain patient dignity, comfort, and safety at all times

Qualifications

• Current, unrestricted LPN or RN license (state-specific)

• CPR/BLS certification (ACLS preferred for RNs, if applicable)

• Experience with ventilator-dependent patients and tracheostomy care required

• Experience with G-tube care and enteral feeding

• Ability to provide total care for a medically complex patient

• Strong assessment, critical-thinking, and documentation skills
