Care Finders Total Care, a Modivcare Personal Care Service, is looking for an RN or LPN responsible for providing high-quality services to our home care private duty clients.


We are seeking a compassionate, skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide one-on-one, private duty nursing care a 9-year-old girl in a home setting. She is a sweet child with complex medical needs who thrives with consistent, attentive, and loving care. This role is ideal for a nurse who values relationship-based care and is comfortable managing advanced pediatric medical conditions.

• Pay: LPN hourly rate $35 - RN hourly rate $45
• Schedule: 

  Tuesday and Thursday 6:30am-3pm; Saturday and Sunday 20 hours/day

Patient Profile

Our patient is a 9-year-old female with a history of:

• Cardiac arrest

• Tracheostomy with ventilator dependence

• Gastrostomy tube (GT) for nutrition and medications

• Organic brain disease

• Global developmental delays

She requires total care and close monitoring but is medically stable with a structured routine and attentive nursing support.

Responsibilities

• Provide skilled nursing care in accordance with the patient’s individualized care plan

• Manage and monitor tracheostomy and ventilator, including suctioning and emergency response

• Administer medications, feeds, and hydration via GT

• Monitor vital signs, respiratory status, and overall condition

• Recognize and respond promptly to changes in condition or medical emergencies

• Perform routine assessments and accurate documentation

• Assist with positioning, transfers, and personal care

• Maintain a clean, safe, and supportive care environment

• Communicate clearly and respectfully with family members and healthcare providers

• Offer comfort, reassurance, and developmentally appropriate interaction

Qualifications

• Current RN or LPN license in good standing

• Pediatric nursing experience preferred

• Experience with trach/vent and GT care strongly preferred (training may be provided for the right candidate)

• Current CPR/BLS certification (PALS preferred)

• Ability to remain calm and confident in emergency situations

• Reliable, punctual, and professional

• Physically able to assist with lifting, positioning, and extended bedside care
