All Metro Health Care, a Modivcare Personal Care Service, is looking for a Director of Nursing responsible for directing our clients nursing services provided by our Licensed Home Care Service Agency (LHCSA) and works in collaboration cross functionally with team's throughout the company.  

  • Pay: $120,000 - $175,000, based on experience
  • Bonus: Eligible yearly based on individual performance 
  • Bonus: Eligible yearly based on company performance 
  • Schedule: M-F 8:00AM-5:00PM 
  • Candidate may be outside the NYC area, but must be comfortable with traveling
  • Travel: Downstate/Upstate NY offices as needed
  • Main Office: 70 Sunrise Hwy suite 520, Valley Stream, NY 11581

You will... 

  • Provide strategic leadership and direction for nursing, ensuring exceptional client services.
  • Ensure compliance with regulations, laws, and organizational policies.
  • Maintain adequate staffing with qualified personnel to meet patient needs, standards, and goals. 
  • Foster collaboration and professional growth through impactful meetings and team development.
  • Coordinate the Quality Improvement Program for compliance and best practices.
  • Conduct audits, inspections, and implement corrective actions as needed.
  • Monitor patient outcomes and implement strategies for enhanced care.
  • Address patient concerns and support operations in addressing trends.
  • Support contract negotiation and identify opportunities for engagement.
  • Perform nursing team functions, including home visits when necessary.
  • Define annual Key Performance Indicators aligned with corporate goals.
  • Manage budgets and optimize operational performance.
  • Travel for business purposes.


We are excited to speak to someone with the following… 

  • Registered Nurse (RN)
  • 5+ years of community health and/or home health, preferably personal care and private duty nursing required
  • 5+ years of Licensed Home Care Agency (LHCSA), Certified Home Health, or community hospice experience highly preferred
  • 5+ years of leadership or management experience required
  • 5+ years of leading/managing multiple site nursing teams
