Per Diem RN Supervisor – Home Care
Binghamton, NY & Surrounding Areas | Flexible, Visit-Based Schedule
All Metro Health Care, a Modivcare Personal Care Service, is looking for an experienced Per Diem RN Supervisor to support our home care clients and caregivers in the Binghamton area. In this role, you’ll provide clinical oversight, conduct home visits, and mentor Home Health Aides while helping clients receive safe, high-quality care in the comfort of their homes.
Pay & Position Details:
Pay Per Visit:
- Supervisory/Post-Hospital Visit: $60
- Reassessment: $75
- Initial Admission: $90 weekday / $125 weekend
- Schedule: Visits planned with the required presence of the patient's aide.
- Mileage reimbursement at the current federal rate

What You’ll Do:
- Evaluate, educate, and support caregivers on assigned cases
- Provide clinical orientation and participate in training and in-services for new nurses and caregivers
- Conduct initial assessments and supervisory home visits
- Assists physician in developing a plan of treatment/care and update plans of care based on client's needs/condition.
- Counsel clients and caregivers regarding nursing and care needs
- Complete clinical progress notes and report changes in client condition
- Review clinical records for accurate and complete documentation
We’d Love to Meet You If You Have:
- Active NYS Registered Nurse (RN) license
- CPR certification
- 1 year of home care experience required
- 1 year of supervisory or management experience required
- Valid NYS driver's license, insurance, registration, and reliable vehicle
