Description
All Metro Health Care, a Modivcare Personal Care Service, is looking for a On Call Care Coordinator Associate. You will have the opportunity to make a positive impact on the lives of our patients by ensuring their care needs are met effectively and efficiently.
- Pay: $18.00 per hour for time worked
- Weekday/Friday shifts: Minimum 4 hours of pay guaranteed, even if only one phone call is received
- Saturday & Sunday shifts: Minimum 8 hours of pay guaranteed for monitoring phones during the 24-hour coverage period
- Schedule: This role provides coverage for after-hours calls, ensuring callers receive support when the office is closed.
- Friday: 5pm - 8am
- Saturday- 8:00 AM – 8:00 AM (overnight, 24 hours)
- Sunday- 8:00 AM – 8:00 AM (overnight, 24 hours)

You will...
- Assign field staff schedules and confirm assignments with employees and patients.
- Speak with patients or patient families regarding schedules, call-offs, cancelations, and appointments.
- Document client data and visits in the scheduling system
- Be the backup to the Intake department for receiving and coordinating home care service requests.
- Understanding that communication is vital within the office as well as to everyone involved in our clients' care.
We are excited to speak to someone with the following…
- High School Diploma or GED
- 2+ years of customer service experience required.
- 1+ years of home healthcare experience preferred.
- Electronic Visit Verification (EVV) system experience preferred.
